All fields must be completed in BLOCK CAPITALS. Full payment is required to confirm your booking. Unpaid applications will not secure a space. No refunds are provided for cancellations, including missed sessions for any reason. Sessions cannot be rescheduled or transferred. It is your responsibility to ensure timely arrival and that the participant is appropriately attired. Swimsaga reserves the right to deny participation due to inappropriate or unsafe behaviour. In the rare event that a session is cancelled by Swimsaga, an alternative date or credit will be offered. One form must be completed per child.
